Instructions: The table below lists community-based economic empowerment partners or service providers that can support your program participants. For each line, select the answer that best describes your organization’s relationship with the provider type. This is a comprehensive list; it is likely that your program would benefit from collaboration with some, but not all, of these partner types. Responses to each row are not required.

What do we do? What activities are part of our economic empowerment program?
Identify the core services delivered to meet your program’s objective. This may include case management services, financial classes, job training/placement, direct financial services.
Step 2: For each activity, what results do I expect from my program’s services?
For each activity, identify the outcomes for your program, meaning the specific changes or results you expect the program to help participants or communities achieve. There may be more than one outcome per activity. The outcomes can include changes in attitudes, behaviors, knowledge, skills or situation. Be sure that the outcomes are achievable for your participants and something your program can realistically help change.
